CDC's National Center for Health Statistics (NCHS) is the nation's principal health statistics agency.
CDC PLACES
PLACES, a collaboration between CDC, the Robert Wood Johnson Foundation, and the CDC Foundation, provides model-based population-level analysis and community estimates to all counties, places (incorporated and census designated places), census tracts, and ZIP Code Tabulation Areas (ZCTAs) across the United States.
CDC WONDER
CDC Wonder offers demographic statistics for the US, including births, deaths, and for certain conditions and diseases like AIDS and cancer.

data.census.gov

data.census.gov provides data from the U.S. Census Bureau including the 2018 American Community Survey, 2017 Economic Census, 2020 Census and more.
PolicyMap
Policy Map is a web-based Geographic Information System that provides a simple mechanism for visualizing data from 2000s to the present. Available data includes demographics, home sale statistics, health data, mortgage trends, school performance scores and labor data like unemployment, crime statistics and city crime rates.
Look in the Health and Quality of Life categories to map variables related to environmental health disparities.

Dartmouth Atlas of Health Care
No longer being updated as of 6/2024. Dartmouth Atlas Project documents variations in how medical resources are distributed and used in the United States. The project uses Medicare and Medicaid data to provide information and analysis about national, regional, and local markets, as well as hospitals and their affiliated physicians.

Community Health Database (CHDB) Tool
The PHMC CHDB Tool allows users to do queries of the Southeastern Pennsylvania Household Health Survey by geographic region (county, ZIP code), key health indicators, and demographic characteristics (race/ethnicity, age, gender, health insurance status, poverty status).
