This page includes publicly available datasets that can be used for community engaged research. The data can also be used for other purposes such as in the background section or justification for a grant proposal. The data is freely accessible for community organizations, community partners, and researchers alike. Community Health Needs Assessment
The CHNA is a requirement for tax-exempt hospitals to engage their servicing communities every three years and assess their health needs. Based on their findings an action plan is developed to work alongside community stakeholders in improving the health needs of the communities.
